Feed Order Form Instructions

As a convenience to our customers, a feed order form is now available to place orders for feed to be delivered from the Staunton Feed Mill. To e-mail the feed order form, please complete the following steps.

• Click here to access the feed order form.
• You must complete all fields that are ORANGE in color. These are required fields. The form will not be submitted if all required fields are not completed.
  • When entering the phone number, enter the 10 digit number only. For example, phone number (555)444-3333 would be entered as 5554443333. Do not enter any letters or symbols.
  • When entering the date of delivery, simply select the date from the drop-down box. All dates are in the form of MM-DD-YYYY.
• Select the feed you want by using the drop-down box under the appropriate animal species. If you want a custom feed formula that has been set up for your farm, go to the custom feeds box. Select "Enter Custom Feed" from the drop-down list. Then type in your custom feed name in the box.
• Enter the quantity needed for each feed selected. (If you want the feed in bulk form, enter the # of tons needed in the bulk column. If you want the feed in bagged form, enter the # of bags needed in the bagged column.)
• Once the form is completed, click the "E-Mail Form" button. The form will then be e-mailed to Augusta Co-Op through your computer e-mail program (Microsoft Outlook, Outlook Express, Apple Safari, Mozilla Thundebird, etc.) You will receive a reply e-mail back that Augusta Co-Op received your feed order during normal business hours. Feed Orders must be received 48 hours before delivery.

There is also an option to print a copy of the form for your records before submitting it by e-mail. If you make an error while filling out the form, you can click on the "Reset Form" button to start over.
