Augusta Co-Op Forms

The heart of any cooperative is the members that support it! We welcome new members every day to Augusta Co-Op. If you produce agriculture products and are interested in becoming a new member, please download and fill out the form(s) below. ALL FORMS MUST BE SIGNED and either dropped off or mailed to the main office in Staunton.

  • Membership Application - This form needs to be completed by any person or business entity that produces agriculture products with gross sales of $1500 or more per year. Once your agriculture membership application is received, you will be given a co-op membership number, which you will provide at the time of purchase.

  • Credit Application (2 pages to complete) - This form needs to be completed by any person or business entity that desires to have a co-op credit account set up under their co-op membership number, This form may be completed by co-op members as well as other patrons that shop the co-op such as individuals, civic organizations, government agencies, etc. A credit account should be set up for fertilizer, crop protectant, farm supply and feed delivery services. By setting up a credit account, it is easier for both our agriculture members and our other patrons to make and track purchases made at the co-op.

  • Feed Order Form - As a convenience to our customers, a feed order form is now available to place orders for feed to be delivered from the Staunton Feed Mill.
  • 4-H/FFA Application Form - This form is for youth that desire to set up a credit feed account for their 4-H & FFA show animal projects.
  • Virginia Sales Tax Exemption Forms - There are several forms for sales tax exemption on retail sales in Virginia.  Please choose the form below that best matches your individual situation.
    • ST-10       For RESELLERS that purchase items that they intend to resale at retail
    • ST-11       For MANUFACTURERS purchasing items that are used in the manufacturing of a retail product
    • ST-12       For GOVERNMENT AGENCIES that purchase items for their use at Augusta Co-Op locations
    • ST-13A     For CHURCHES who purchase items for their use at Augusta Co-Op locations
    • ST-18       For FARMERS purchasing items used directly in the production of agricultural products for sale

 

If you do not qualify as an agriculture member and do not desire to set up a credit account, you can still get an account number set up with the co-op. With this type of account, payments are made at the time of purchase by cash, check or credit/debit card. Many individuals and organizations desire this option to make it easier to track their co-op purchases and to receive notices about special events. Please contact the co-op main office directly at (540)885-1265 ext 238 to discuss this option.

You do not have to be a member to shop at the co-op!
